What Is a Varicocele?

Varicocele is the swelling or enlargement of veins in the scrotum (the loose skin covering the testicles). It resembles the varicella seen in the legs.

These enlarged veins can cause blood to pool, raising the temperature around the testicles. The link between varicocele and male infertility stems from this increased heat and reduced blood flow, which may affect:

  • Testicular function and health
  • Healthy sperm production
  • Sperm count, motility (movement), and morphology (shape)
  • Sperm DNA integrity

Common Varicocele Symptoms

  • A dull, aching pain or heaviness in the scrotum (often worsening after standing or physical exertion)
  • Visible or palpable enlarged, twisted veins (often described as feeling like a “bag of worms”)
  • A noticeable difference in size between the testicles
  • Difficulty achieving pregnancy with your partner despite no other identified causes

How Is a Varicocele Diagnosed?

  • Physical Examination: A physical check by a specialist while you are standing and relaxed is the gold standard for detecting a clinical varicocele.
  • Semen Analysis: Evaluates sperm count, motility, and shape to see if the varicocele is affecting your semen parameters.
  • Scrotal Doppler Ultrasound: Used when clinically indicated to confirm vein diameter and retrograde blood flow.
  • Hormonal & Fertility Tests: Additional blood tests assess testosterone and other reproductive hormones when necessary.

When Does a Varicocele Need Treatment for Infertility?

According to joint guidelines from the American Urological Association (AUA) and the American Society for Reproductive Medicine (ASRM), surgical treatment is generally considered only when all the following criteria are met:

  1. The varicocele is palpable (felt during a physical exam).
  2. The couple has documented difficulty conceiving.
  3. The male partner has abnormal semen analysis parameters.
  4. The female partner has normal fertility or potentially treatable fertility factors.

A “subclinical” varicocele (one seen only on ultrasound but not felt during an exam) does not routinely require surgical repair.

Varicocelectomy for Male Infertility

Varicocelectomy is a surgical procedure designed to tie off or redirect blood flow away from the enlarged veins, restoring normal temperature and circulation to the testicles.

  • The Goal: It aims to improve the testicular environment, supporting healthier sperm production over time.
  • Timeline: Because a full sperm production cycle takes about 70 to 90 days, improvements in semen quality typically take 3 to 6 months to appear.
  • Realistic Expectations: While varicocelectomy often improves semen parameters in well-selected candidates, individual responses vary. It is not an automatic guarantee of pregnancy, which is why a holistic fertility approach is always necessary.
